How to Help Ensure a Successful Live Scan


It's important to prepare for your Live Scan fingerprinting appointment in advance by having all your materials ready. Knowing what to bring with you is essential. Here are 3 things to do now to prepare for your Live Scan appointment.


Step 1—Ask for a Live Scan Request Form

You may have been asked for a Live Scan background check from an employer, license issuer, law enforcement agency, nonprofit organization or other relevant certifying body. This is called the requesting agency, and they provide a Request for Live Scan Service Form.


The nature of the business, agency or other organization will dictate the background check requirement, and determine whether or not you need electronic fingerprinting via Live Scan. No two requesting agencies are the same, and only the requesting agencies will know what their own background check requirements are. As a first step, you need to ask for the correct Live Scan form from your requesting agency.


Step 2—Complete the Live Scan Request Form


Make sure to fill out your Request for Live Scan Services Form as completely as possible before you arrive at J&H Live Scan Service Providers for your appointment. It's important to understand that our Live Scan technicians will not know the information requested for the form. We cannot help you complete the Live Scan Request Form.


You will need to provide your personal contact and identifying information including your legal name, gender, birthdate, address, etc. Also keep in mind that the requesting agency's Originating Agency Identifier (ORI) and mail code must be on the form. Your requesting agency should provide you with this information, but make sure to confirm with them that this information is present and correct. The ORI is the code assigned to that agency by the California Department of Justice.


Step 3—Book Your Live Scan Appointment


Make sure to fill out your Request for Live Scan Services Form as completely as possible before you arrive at J&H Live Scan Service Providers for your appointment.
